I AM SUBMITTING MY CLAIMS TO BLUE CROSS BLUE SHIELD THROUGH A MODEM WITH AVAILITY. IS THEIR A WAY TO ATTACHED A LETTER OF MEDICAL NECESSITY? BLUE CROSS BLUE SHIELD HAS BEEN REQUESTING THIS LT AND I HAVE BEEN SUBMITTED THESE CLAIMS ON PAPER.

Your record indicates that you have the Enhanced Direct Billing module which should give you the option to attach a CMN form from the Charges screen. A CMN is the only documentation that may be attached with your electronic claims.

I CHECKED THROUGH ALL OF THE CMN DATA AND THERE IS NOT ONE THAT I CAN USE FOR PROSTHETICS AND ORTHOTICS. IS THERE ANY OTHER THAT I CAN DOWNLOAD?

There is not one specifically for oththotics & prosthetics, but the generic CMN has successfully been used for that purpose.

I did see a printable CMN form, but, how can I get that form where I can attach it to a electronic claim. It is not in my list of forms to attach?

Only CMN's that are in the list can be attached in an 837 format.

Generic CMN's cannot be cannot be attached and sent in an 837 format.
